The solution
I designed a suite of printed handouts, handbooks and PowerPoint presentations across two distinct needs: clear, accessible disease information for patients, and practical transition support tools to help young people build the confidence and skills to manage their condition independently. The materials were designed for both facilitated use with a Healthcare Educator and self-directed completion.
